Job Description
Creates and maintains SISU Patient Visit schedules for Nurses. Patient visits are for the Home and the AIS (Ambulatory Infusion Suite) settings.
Arranges and organizes patient schedules in line with visit frequency in the Nursing POT (Plan of Treatment) and in accordance with new patient orders. Considers patient availability and Nursing Staff availability when generating the schedules.
Maintains adequate time allotments for patient visits to assist in Nursing Work time efficiencies and control of overtime.
Responsible for properly entering patient and staff information into SISU scheduling system and updating it on a regular basis or when changes occur, to ensure the accuracy of the scheduling system.
Reaches out to patients and nurses with regards to the schedule or any changes in the schedule.
Notifies the Pharmacy and Intake personnel of the Patient / Nursing Schedule or other team members to ensure care coordination.
Assists the billing department by entering and confirming all Nursing Visits into the Clinician Log in CareTend in a timely manner.
Seeks the assistance of Subcontracted Nursing Agencies to cover SISU patient visits in the event that SISU Nursing is not available. Follows the Nursing Agency referral and authorization process for this type of Nursing Coverage.
Establishes and maintains accurate and complete patient records. Documents all communication with patients and team members.
Keeps the Nursing Manager, staff nurses and patients updated in regards to any scheduling conflicts.
Participates in surveys conducted by authorized inspection agencies.
Participates in the organization’s Performance Improvement program as requested by the Performance Improvement Coordinator.
Participates in organization committees when requested.
Participate in in-service education programs provided by the organization.
Perform other duties as assigned by supervisor.
Education
High School graduate or equivalent level of education required.
A post-secondary school degree or certification in a health-related field is advantageous.
Experience & Knowledge
Ability to organize and plan the Patient / Nurse Schedule.
Organizational skills sufficient to maintain consistently accurate records.
Ability to evaluate options and to make efficient decisions.
Computer skills that include proficiency with spreadsheets and use of the internet, as well as word processing, and efficient use of the internet and e-mail.
Ability to prioritize in the face of multiple tasks or duties.
Minimum of 2 years’ experience in Home Infusion Scheduling.
Must possess excellent communication and customer service skills.
Must have a working knowledge of medical terminology.
Must have knowledge of general office practices and procedures including filing, record keeping, scanning, photocopying, and faxing.
Must be able to follow a defined list of tasks and follow the established procedures completing them in a timely fashion.
Must be able to flex with the increased workflow when census is higher and recognize urgency of each task.
Skills
Must have working ability in Microsoft 365, Office and Teams.
Must have excellent communication skills and phone presentation in interactions with patients and staff.
Must be able to prioritize workload, work with minimal supervision, organize and complete tasks accurately and in a timely manner.
Must be able to organize varied and possibly conflicting time constraints with calm reaction to frequent and often last-minute schedule changes.
Must be able to exercise good judgement related to urgent staffing needs for safe care.
Ability to maintain harmonious constructive relationships with internal and external customers.
Demonstrated skills in effective written and verbal communication.
