• 1095-C (Employer Coverage Offer): Log in to myADP.com under Tax Statements

• 1095-B (Proof of Coverage): Log in to myuhc.com under Tax Documents. If your address on file is current, UHC mailed you a copy by January 31st. To request a paper copy of your 1095-B, call customer service at 1.800.767.0700.
